Maternity Outpatients Self Referral Form

To make your first booking appointment (which should ideally take place between 6-12 weeks of pregnancy) please complete the: Maternity Outpatients Self Referral Form. It is important that you refer for maternity care as soon as possible after finding out you are pregnant to ensure we can give you an appointment at the right time. Your request will be processed and you will be contacted within 4 weeks.

If you think you are 10 weeks pregnant or more, please do not complete this form and contact Maternity Outpatient’s Department at Leighton Hospital by telephone on 01270 273127, Monday to Friday 8.30am – 4.30pm (excluding bank holidays).

Your first booking appointment will take approximately an hour and a half to gather as much information about you, to plan your care appropriately.

If you are pregnant and have concerns up to 16 weeks, you may be able to be referred to the Early Pregnancy Assessment Unit. More information about the service and how to be referred can be found on their website page.

2022.06.pngBased at Leighton Hospital, we have both Consultant Led and Midwifery Led Units, providing care in a semi-rural area undertaking approximately 3,000 births per year.

You may also wish to consider giving birth at Macclesfield District General Hospital or Royal Stoke Hospital. It is important that you see a Midwife as early as possible in your pregnancy. 

We give you the information that you need so you can make informed choices about your care throughout pregnancy, labour and birth. We will always ask for your consent for any type of medical treatment, test or examination throughout your pregnancy, labour and birth. Ward relocations

From Monday 22nd January 2024:

During essential maintenance some of our maternity services will be relocated. We apologise for any inconvenience caused.

Both Midwifery Led Unit and Triage will be located on Ward 24. To access, use the Pink Zone Entrance and proceed along the pink corridor, take the lift/stairs on the left opposite Ward 22 to the first floor. Turn right when exiting lift/stairs to Ward 24. Ward 24 is located next to the Maternity Ward (Ward 23).

All telephone numbers will remain the same during the relocations.

Conducting quality research is essential to providing evidence-based care with the aim to improve patient care. Leighton Hospital Maternity Unit has been a research active site for many years.

Midwives work jointly with the Trust’s Research and Development Team to encourage a research culture within the unit which promotes service development and provides the opportunity for women to participate in ethically approved studies. Research midwives and a Paediatric research nurse support the Women and Children’s Division, recruiting to a wide range of reproductive health and paediatric studies.

In addition to your normal hospital care you may be invited to take part in one of the many research studies currently being undertaken at the Trust.

Clinical research helps us to improve and develop the care that is given to women, their babies and children. This could be a drug trial, a screening study, a questionnaire or a combination of these and your participation is entirely voluntary.

We appreciate the help and support we have received from women and their families, both in the past and currently.
