Mid Cheshire Hospitals NHS Foundation Trust’s Frail Elderly Assessment Unit (FEAU) recently celebrated its first year since opening, marking a significant milestone for the teams involved.

The unit, located at Leighton Hospital, opened in April 2023 and is focussed on preventing hospital admission by supporting some of our local communities most vulnerable patients to get better, providing early access to rapid investigations and treatment enabling them to be discharged to continue their care in the most appropriate place.

By pro-actively reviewing and identifying patients first thing each day to be moved to the unit ready to be reviewed by a Multi-disciplinary Team of specialists including a dedicated prescribing pharmacist, discharge nurse and physio/OT resource, the unit has successfully discharged 1,289 patients over the past 12 months.

This has saved the organisation approximately 6,445 bed days and has ensured our patients are being cared for in their local community with a team of health and care professionals wrapped around them to support them to enjoy their life to the fullest.
