Our Advanced Paediatric Nurse Practitioner service is provided by a specialist nurse who provides highly specialist input, support and co-ordination of care. The role requires close collaboration with a number of interdependent services and professionals, working within complex systems including acute care, primary care, schools, communities. The service will aim to provide specific interventions to attempt to reduce hospital admissions and GP appointments specifically in the areas of Eczema and paediatric respiratory conditions.
The aims and objectives of the role are:
• To empower families to self-manage their children’s conditions through provision of health education, advice.
• To work with primary care to coordinate regular reviews of identified patients who frequently access acute services, in order to reduce presentation at accident and emergency and avoid hospital admission.
• To reduce the need for patients to present for emergency GP appointments through timely assessment, intervention and advice.
• To liaise with health visitors and school nurses and other care providers e.g. schools to ensure a consistent approach to support and advice to the identified children and families
Our APNP service is also currently heavily involved in the Child Health Hub project within the Cheshire East Place, and is contributing to service development through:
- Development of Universal Asthma training for service users, practice nurses, school nurses
- Follow up of asthma / viral wheeze discharges from MCHT within 48 hours (virtually, and then face to face If required)
The APNP receives referrals from all health professionals, but specifically Health Visitors, GPs and Paediatricians. Referrals are accepted for children and young people, aged 0-16, in the following segments:
- Children that have had more than 2 admissions in 6 months or 4 or more GP consultations within 3 month period presenting with respiratory disorders defined or eczema
