The Ambulatory Wound Care Service is available to patients who have wounds that require specialist support and intervention.
The wound care team is made up of registered nurses who are highly skilled professionals trained in wound management. The team deliver a high standard of quality care, supported by Central Cheshire Integrated Care Partnership (CCICP) standards and evidence based practice.
A 30-minute appointment will be booked via telephone with the administration hub to attend one of the clinics. The team will try to get you a clinic space as close to your home as possible.
A full holistic assessment will be undertaken by a trained healthcare professional. This will consider physical, social and emotional needs.
If you require a follow up and referrals to other services, this will be discussed at your appointment.
Where appropriate, a self-care approach to wound management is adopted. The team will support you to do this and provide you with the resources and information to do so. The team have developed a self-care pathway with digital technology to support this.
The service is located within the boundaries of the 5 care communities under CCICP, clinics are located at:
Northwich – Victoria Infirmary
Winsford – Dene Drive Medical Centre
Crewe – Eaglebridge Well Being Centre
Nantwich – Church View Primary Care Centre
Sandbach – Ashfields Medical Centre
Alsager – Cedars Medical Centre
The clinics operate 9-5 Monday to Friday with one clinic running at a weekend in Northwich only.
We will try to accommodate you at the clinic nearest to your home address.
An appointment is required to attend the service, this isn’t a walk in service.
Patients need to be registered with a GP surgery within the 5 care communities across CCICP.
Patients living in the South Cheshire area will be required to see the practice nurse for 2 weeks prior to being referred to the service.
Patients in Vale Royal can self refer by contacting the Northwich / Winsford admin hubs.
