Central Cheshire Integrated Care Partnership's Nutrition and Dietetic Service operates across South Cheshire and Vale Royal. The service is based at Mid Cheshire Hospitals NHS Foundation Trust.
The Dietetic Service is diverse, providing input across all clinical specialities and settings including inpatients and outpatients, General Practice surgeries and nursing homes.
The service objectives

To provide specialist inpatient care and outpatient clinics to provide therapeutic dietary advice as an integral part of disease management

To maximise nutritional wellbeing in hospital to facilitate reduction in length of hospital stay and prevention of readmission

To advise and support appropriate cost effective prescribing of nutritional products across acute and community settings

To assess, advise and provide ongoing support and equipment to adults and children who are tube fed
-
To provide group education to individuals diagnosed with Type 2 Diabetes and carbohydrate counting group education for individuals with Type 1 Diabetes

To provide a range of training programmes to medical and nursing staff.

The Nutrition and Dietetic Team provides individualised dietetic intervention to a wide ranging client group. The dietetic teams are:
Hospital Team
Hospital dietitians provide specialist dietary advice to help patients to manage their medical condition and maintain nutritional status while in hospital and to promote recovery. Dietitians work closely with multi-disciplinary teams such as paediatrics, diabetes, critical care, gastroenterology, stroke and oncology The dietitians also provide general and specialist outpatient clinics follow up, if required, on discharge and following consultant / GP referral.
GP Practice Team
GP dietitians provide an outpatient service for patients to attend in their GP practice. Within these clinics the dietitian provides advice on therapeutic diets for disease management.
Home Enteral Feeding Team
This team provides tube feeding and nutritional support advice, to support food first advise, to patients in nursing homes and their own homes.
Specialist Weight Management Team:
Pre-bariatric surgery assessment and support.
The weight management team provides community based clinics to assist patients in achieving weight reduction and weight maintenance, prior to referral for bariatric surgery. NHS England set specific criteria for referral bariatric surgery; the referral data required is included on the specific specialist weight management referral form to be completed by a patient's GP. The dietitian will explain the referral criteria for surgery at the initial appointment.

How can I be referred to a dietician?
Individuals can access the services of a dietitian through their GP or hospital consultant. The doctor will complete a referral form or letter providing relevant medical information. This helps the dietitian to complete a detailed dietary assessment and provide appropriate dietary advice.
What to expect and bring to first appointment
The initial appointment with the dietitian will last approximately 30 minutes. During this appointment the dietitian will generally take a detailed diet history and ask questions regarding any past medical history and current medication. This will enable the dietitian to provide appropriate dietary advice.
To assist the dietitian, we ask that all patients bring an up to date record of their current prescribed medications and any relevant medical information.
If a follow up is required an appointment will be made for approximately 15 minutes.
What if I cannot make my appointment?
If the appointment time is inconvenient or you are unable to make the appointment date, please contact us as soon as possible and we will rearrange or cancel the appointment .
