The Paediatric Occupational Therapy Functional Skills and Motor Co-ordination Service is a tiered service. Children with difficulties relating to functional skills and motor co-ordination will benefit from using the universal strategies and advice as detailed below.
Universal Tier
Please use the strategies and activities (detailed in the ‘Universal Information' section below).
Please complete this for a minimum of 12 weeks prior to considering whether a referral is required.
Please use this diary sheet (this will automatically download as a Word document when clicked) to record the activities and strategies used to address the child’s/young person’s needs.
Please consider who is completing the diary sheet. It may be that Parents/Carers and/or Education Staff are best placed to complete this record, depending on the area of functional difficulty.
See diagram below for guidance or contact the Advice Line detailed in the ‘About This Service’ tab above.
Targeted Tier
Referrals for targeted input can be made via this online referral form (right click & select 'open in new tab' to keep browsing this page) with full parental/carer consent.
Alongside this online referral, a completed diary sheet must be sent via email to ccicp.
Please consider that children/young people should only be referred if they are able to engage in the assessment process.
The targeted tier involves a parent/carer telephone appointment.
Incomplete/insufficient referrals, including those sent without accompanying diary sheet, will not be accepted and will be returned to the referrer.
You can also email other supporting information, such as clinic letters, EHCPs, hospital therapy discharge care plans, and/or other professional reports (with parental/carer consent). Please ensure that the child’s/young person’s full name and date of birth are on each document.
Specialist Tier
Following completion of the universal and targeted tiers, the Occupational Therapist may offer specialist tier intervention. This is likely to involve a clinic assessment appointment and could include standardised assessments, such as motor skills and handwriting assessments, as clinically indicated.
The Paediatric Occupational Therpay advice line is available to parents/carers, health professionals and educational professionals.
This is an opportunity to speak directly to an Occupational Therapist for discussions around the referral process and/or further direction in relation to the universal tier of the Functional Skill and Motor Co-ordiation Difficulties Service.
The advice line 07584 386880, is open from 12.00pm till 2.00pm every Wednesday during Term Time.
