We understand that you may require additional support or for us to make changes to make the procedure easier for you and your carer/support person. 

This could be to do with previous problems with procedures, your health, social circumstances, mental health, learning difficulties, physical ability or language barriers.

Any reasonable adjustment requests will then be discussed with the nurse management team before the scheduled appointment.  You will be notified beforehand if for any reason your requests cannot be accommodated and the reason why.
